There are different types of breast lifts depending on the grade of the breast ptosis (sagging). If the nipples are pointing down (grade 3 ptosis), it is likely a classic lift with an incision around the areola, down the midline of the breast and under the breast along the inframammary fold will be needed to be able to bring the nipple up to the proper position.  This incision is commonly called anchor shaped.

If the nipples are not as droopy (grade 1 or 2 ptosis), it is possible that only a donut mastopexy will be needed to raise the nipples into the proper position.  This procedure, called a Binelli mastopexy, only requires an incision around the areola.  Often times the areola has become larger due to pregnancy and breastfeeding.  The size of the areola can be reduced, if desired, and the nipple is raised to its proper position.  This usually leaves a larger area to be sutured, so the skin is senw in a pleated fashion.  This pleating completely smooths out over time giving a nice result (and virtually scarless).

When a mastopexy is needed, only rarely can an implant alone correct the ptosis because the cause of the sagging is too much skin above the areola.  Even with an implant, only an extremely minor ptosis can be corrected with an implant alone.
